So my current boss has a new role/title. It's not quite the one he interviewed for and it seems an external candidate has been offered the role. Would it be considered "bad form" to send a congratulations note?

I wouldn't bother with an e-mail. Maybe a Congratulatory coffee or donut? If you've got a good relationship, come in tomorrow with a token "good for you!" offering. Congratulate, talk about the promotion and get to work. If the promotion is more of a lateral move or a disappointment, you've got a nice chance to say "Um, congrats? Hey, you'll be great in this new role too".
An e-mail seems a bit cold or like you aren't able to face him. Maybe the new role has suddenly become a great thing - and you are missing out on the opportunity.
But I'm a Sky High I, so YMMV.
